Director of Patient Care (Community and Primary Care)

Salary Scales:

Band 8D - £67,247 - £83,258 per annum

Band 9 - £79,415 - £100,431 per annum

The Trusts are now seeking an experienced Director of Patient Care (Community and Primary Care) to join this collaboration and work strategically to provide a great service for patients in the Somerset health and care system.

In this role, you will lead and play an integral role in the implementation of the Trust’s strategy and be at the forefront of transforming and modernising our mental health services across Somerset.

You will deliver improved outcomes and satisfaction for patients and service users through a person-centred approach to service provision and transformation. Using innovation and quality improvement methodology, you will transform community services and achieve the triple aim of improved physical and mental health, better care and lower cost.

You will have demonstrable evidence of:

    • A clinical background with experience in a senior leadership role
    • Successful management of complex organisation-wide change
    • A positive value base that supports the empowerment of colleagues and patients
    • Enthusiasm, passion and energy that will enable you to lead and motivate our workforce to co-produce, with key stakeholders, the service developments required
