Band 4 - Patient Care Co-ordinator

The purpose of this post is to actively manage the administrative processes of patient pathways, ensuring a smooth and efficient service for patients throughout their treatment by the Trust. Also to ensure and participate in providing an effective administration service to the multidisciplinary team.

We are looking for an exceptionally organised individual who is able to work proactively and independently to ensure targets are met. Excellent communication skills, to build and maintain a variety of working relationships is essential. Good computer skills, audio typing skills, processing G2/CRS letters and knowledge of using the CRS systems to book outpatient appointments is key in this role as it consists of part secretarial duties and part pathway management duties; which consists of booking/rebooking outpatient appointments and general management of patient pathway on CRS system. Internal as well as Trust scheduling training will be provided.
