Exciting opportunities have arisen for Support Workers to join Swindon Support Service in Swindon. You will form part of a dedicated team responsible for supporting people with disabilities with all aspects of their daily lives. You will provide support across a supported living household, independent flats and support for service users in the community with activity based support.
You will have excellent communication and interpersonal skills, a friendly approach and the ability to work in an ever-changing environment. You will provide continuous assessment of service users' needs and monitor the aims, objectives and outcomes of the Support/Care Plan along with the ability to accurately complete forms and maintain records.
Under the guidance of the Line Managers, you will be responsible for ensuring the support functions effectively and develop the skills of the people you support as part of their journey to independence as follows:
* being supported to be the best they can be - as independent as possible
* being supported to make their own choices about their lives
* living in a way that promotes their privacy and dignity in their home and life
* being part of their own Swindon Support Service household and local community
* being supported to aspire to reach their true potential
* being given the opportunity and support to develop new skills

This is a varied role within a friendly and supportive team and provides opportunities for a variety of training. Successful applicants will be required to attend an induction programme as well as in house, online and bespoke training.
