Do you want to help build a new model of pharmaceutical care in East Lancashire Hospitals? This is an extremely exciting time to join our pharmacy team as following a highly successful pilot East Lancashire Teaching Hospitals NHS Trust has supported a revolutionary roll out of our enhanced model of ward based services that we refer to as the Dedicated Ward Pharmacy model in a phased approach. The aim is that all wards in the Trust have their own dedicated pharmacist with the right level of pharmacy technician support for the level of patient flow through each ward. We currently deliver the model on twenty four wards and we have shown that we can reduce length of stay, get patients home earlier in the day, reduce re-admission rates, reduce drug expenditure and improve patient and staff experience

What does this service mean for a pharmacy technician? You will spend the majority of your working week on either one ward with high patient throughput or two wards with slower patient flow. You will ensure that medicines reconciliation takes place for all new patients and queries are flagged to your pharmacist to resolve during the ward round, that appropriate medicine supplies are made and you will prepare the medicines-related sections of patients’ electronic discharge letters for your pharmacist to authorize. You will also be involved with patient counselling and referring them to their community pharmacist or for a domiciliary visit using the Refer-to-Pharmacy application. You will be the leader of a small team of technicians and assistants and will be expected to offer mentorship, and to support the development and training of junior colleagues.

What else do you need to know about the pharmacy service?

We are a large and forward-thinking department; with extended hours Acute Medical Unit team operating between 7am-11pm 7-days a week. We have a large cohort of Non-Medical Prescribers and are in process of rolling out Electronic Prescribing and Medicines Administration across the Trust. Our ground-breaking electronic bed-board enables speedy communication between ward-based pharmacy teams and we are an early-adopter site for the Royal Pharmaceutical Society’s Transfer of Care initiative. We innovated and are the first site in the world to have Refer-to-Pharmacy . We also currently have the North West region’s only full time Emergency Department (ED) pharmacist working as an advanced (pharmacist) practitioner with their own patient case load in ED-majors. Our Medicines Support Team is managed by the pharmacy directorate. This is a domiciliary team based out of Burnley General Hospital who receive referrals from the hospital, community teams and GPs for patients requiring further support with their medicines post-discharge or to help prevent admissions/re-admissions to hospital.

You may be required to work cross site at our two sites, delivering clinical ward-based services to Surgical, Medical and Family Care divisions. We also deliver training in technical services. The department is also committed to a non-medical prescriber programme.

You will take part in our weekend rota (at a current frequency of around 1 in 8 weekends) and our bank holiday rota (up to two a year)
