£25,314 per annum for 36 hours per week based at Epsom Health & Care Alliance, Epsom General Hospital, Dorking Road, Epsom, Surrey KT18 7EG.
This role involves shift work including weekend working.
The work involves travelling within Surrey and also to other parts of the county. You will therefore need to be a car driver and have access to a car.

About the Role
You will join a multi-disciplinary team of nurses, GPs, community matrons, therapists (physiotherapists and occupational therapists), social care practitioners, healthcare and reablement assistants who together support people over 65 who are either in hospital or at risk of hospital admission, ensuring their care is truly coordinated around their needs and those of their carer.
Your role will be to work with health colleagues and to provide joint health and social care assessment support and regular reviews. Liaising with agencies, providers, voluntary and third sector organisations to ensure local residents receive the right health and care support to safely remain at home.
You will work with individuals and their carer’s ensuring a personalised approach supporting vulnerable adults to identify and arrange services that meet their needs and promote independence, wellbeing, choice and control.

This role sits within the Epsom Heath and Care service team. You will be employed by Surrey County Council, one of the four local organisations that runs the service. Interviews will be with the employing organisation and a representative from one of the other partner organisations.
As a Senior Social Care Assistant, you will be part of a team and will have access to direction, support and supervision from a qualified Social Worker.
You will be supported by qualified practitioners to apply your knowledge and experience of Safeguarding, mental capacity and risk assessment to practice.
Senior Social Care Assistants will have substantial experience of working directly with people with social care needs, including visiting them in their own homes. You will be confident and competent in working with people with a variety of physical and cognitive impairments.
You will need to have competent basic IT skills and be able and willing to learn to use a variety of software systems.
Post holders may work with any combination, or all, of the following care groups: older people, people with physical, sensory, cognitive impairments, people with a learning disability and people with mental health problems.
