An exciting opportunity has arisen for a permanent Band 7 Clinical Practitioner, 30 hours per week, to join our integrated health team based at Shipston on Stour, Warwickshire. The team offers a community nursing service from 8.00am to 8.00pm carried out by qualified and unqualified staff that are supportive, hardworking and passionate about providing the highest quality of care to patients within their area.
The successful candidates will be highly motivated, enthusiastic practitioners who strive to deliver person centred care underpinned by excellent clinical skills and knowledge. They will be responsible for patients within the integrated health team caseload who have complex needs/conditions and who are at risk of deterioration and unplanned hospital admissions, therefore one of the main aims of the role will be to maximise opportunities for patients to remain in their own environment.
The role involves working closely with those patients to carry out a thorough assessment in order to implement a holistic, individualised plan of care to ensure all their needs are met. To support this plan you will also be required to offer clinical leadership and direction to staff within the team, undertake teaching and provide supervision on a day to day basis to increase their skills and knowledge in the management of complex patients.
You must have a minimum of 2 years band 6/7 experience working within a senior clinical role, be able to work independently to make clinical decisions regarding patient care and work closely with GP’s, secondary care and wider multidisciplinary teams. You will need excellent communication and interpersonal skills and have the ability to motivate, influence and inspire new ways of working.
