An exciting opportunity for Band 2 Therapy Assistants has become available to join the transformational Early Supported Discharge (ESD) service at the Royal Berkshire Hospital.
ESD is a Therapy led team (Physio, Occupational Therapists) that provides an outreach service to facilitate timely patient discharges from the Royal Berkshire Hospital.
The service involves discharge visits with patients from the ward to their home; and subsequent follow up rehabilitation focussed follow up calls.The service ‘bridges the gap’ between when a patient is clinically safe to leave hospital and when community teams are available to start on-going care / rehab.
As a Therapy Assistant you will be predominantly responsible for providing follow up visits to patients. You will be expected to participate in weekend working and shift patterns.
Following excellent outcomes with patients from the Hip Fracture Unit (HFU), the team is expanding its service to a wider range of clinical areas across the Hospital. As well as patients on the HFU, it will now be possible to include those from Elderly Care, Medical and Trauma wards.
You will work closely with Senior Therapists and other assistants to meet the needs of the patient group who enter the ESD pathway.
You will need to be enthusiastic, flexible and motivated to contribute to the development of this exciting service. The ability to work flexibly across a seven day, shift work pattern is essential.
The role involves driving on a daily basis and therefore the applicant needs to have a full UK drivers licence.
